In-home Services:
Care coordination is a required component of all In-home services and supports funded under the Expanded In-home Services for the Elderly Program (EISEP) and administered by Nassau County Office for the Aging and its subcontractor agencies. Care coordination is a person-centered planning process that takes place between a qualified case manager/care coordinator, the applicant, and, often, members of the applicant’s informal support network. Care coordination begins with a comprehensive assessment that identifies an individual’s needs, abilities, preferences and available resources. Next, a care plan is developed based on the findings of the assessment. The care plan is a coordinated package of services and supports derived from multiple sources to best meet the applicant’s needs. Finally, ongoing monitoring helps to ensure that the plan is adequate and the program participant is meeting his/her goals and is residing safely in the community.
Eligibility: To be eligible for the program a person must be:
- At least 60 years old;
- Impaired in an "Activity of Daily Living" (such as eating, dressing, bathing or toileting), or in two "Instrumental Activities of Daily Living" (such as meal preparation, housekeeping or shopping);
- In need of assistance;
- Able to be maintained safely at home; and
- Not be a Medicaid recipient. The person who appears to be eligible for Medicaid must be willing to apply for Medicaid. EISEP can only serve this person temporarily while the Medicaid application is being processed.
