Children’s Single Point of Access (CSPOA)

Nassau County Children’s SPOA

CSPOA is a centralized referral system and helps connect youth and their families to supports, services, and programs in Nassau County.  

Nassau County’s Office of Mental Health provides several In-Home programs for children diagnosed with a Serious Emotional Disturbance (SED).  These services are provided through CSPOA.  All CSPOA programs are voluntary.

In-Home CSPOA services:

Some examples of in-home services available include care coordination, family support, skill building, respite, pre-vocational training, and links to other community supports.  All services are provided in the home or community as per family preference. Program serves youths 5 years to 21 years.

If you are interested in referring a youth to CSPOA please complete the following application:

Childrens Single Point of Access Application Part 1

Along with the application please include a psychiatric evaluation completed within the last 12months and a psychosocial evaluation completed within the last 12months.

Once completed please send the materials to CSPOA for review and assignment to the appropriate program. CSPOA@hhsnassaucountyny.us

Youth Assertive Community Treatment (Y-ACT) and Residential Services

Youth Assertive Community Treatment (Y-ACT)-Y-ACT is a community based program that provides mental health services in the home or community.  Services include counseling, medication management, peer services and case management.

Community Residence (CR)- Community Residences are home-like programs for youth with mental health needs. CRs offer supervision, structure, care coordination, skill building and behavior support. The goal of a CR is to assist youth in achieving their best level of functioning so that they can be successful in school and at home when they return to their home and community. 

Residential Treatment Facilities (RTFs)- are therapeutic programs that provide inpatient psychiatric treatment to youth with complex mental health needs and support to their families. The youth have challenges that need around-the-clock supervision, structure, and clinical treatment.
