FOIL Requests

Freedom of Information Request

The New York State Freedom of Information Act provides that the custodial agency may take up to five(5) working days to rule on a Freedom of Information Request (FOIL). FOIL requests are available for viewing or purchasing records.

For FOIL requests, the following forms can be used:

•  Certificate of Examination of Petitions (PDF)
•  Request for Board Material (PDF)
•  Request for Financial Disclosure Statements (PDF)
• Request for Non-Fee Material (PDF)
• Request for Data Processing (PDF)
