What is this survey? 
Nassau County Department of Health Office of Health Equity has contracted JSI, Inc. and Choice for All to conduct a study in Nassau County to better understand Nassau County residents’ views, concerns, and experiences with the COVID-19 vaccine. The survey will take about 15-20 minutes. The survey will ask you questions about yourself, your household, experiences with COVID-19 and the vaccine. To take the survey, you must live in Nassau County and be 18 years old or older.
 
What are the benefits of taking part in the survey?
The survey information helps Nassau County with future planning for COVID-19 and other public health issues. Everyone who takes the survey can enter into a raffle for one of five prizes. The prize is up to $100 worth of groceries of your choice delivered to your home. At the end of the survey, you will have the option to click on a new link to enter the raffle. The information you give to enter the raffle will not be connected with your survey answers.
